Chaperone Coverage

Church Personnel should avoid being alone with minors and, whenever possible, have another adult present or nearby. A recommended ratio of adults to minors for PK-8 is two adults for the first seven (7) minors and one (1) adult for every additional seven minors. Preschools under the jurisdiction of the Office of Catholic Schools shall follow the standard for sufficient staff set forth by that office. The recommended ratio for High School is two (2) adults for the first ten (10) minors and one adult for every additional seven minors. (Roman Catholic Diocese of Tulsa and Eastern Oklahoma. 2024. Code of Ethics Agreement: Child & Youth Protection & General Conduct, Article 4, Section 1, Subsection 1.5.) 

Each adult must have completed VIRTUS training and a background check.

PK-8th Grade
Two (2) adults for the first seven minors and one (1) adult for every additional seven minors. 

High School
Two (2) adults for the first ten (10) minors and one (1) adult for every additonal seven (7) minors.

• The Annual Permission Form & General Release for Minors: Religious Education & Youth Activities form grants permission for the participant to engage in religious formation and youth activities held at the Parish for a twelve-month term, outlining that the parent understands the routine nature of these activities. These cannot be used for any event outside the parish or school. Not even Diocesan or Youth Office events.

• The General Liability Form for Adults & for any Minors Under Their Care form establishes the terms and conditions for participation in the event, including liability waivers, risk acknowledgment, medical consent, media release, procedural matters, and agreeing to be responsible for the for the minors in their care. These cannot be used in place of a General Liability Release Form for Minor Participant. The adult must be present for the duration of the event in order for this to be used for a minor.

• The General Liability Release for a Minor Participant establishes the terms and conditions for the minor participant's participation in the event, including liability waivers, risk acknowledgment, medical consent, media release, and procedural matters. These are only to only be used for a youth’s one off involvements in a parish activity, like youth group, RE class, etc. These cannot be used in place of an Annual Permission Form or Event Permission Form. The parent signing isn’t required to be onsite, but they must be the legal guardian.

• The Local Field Trips, Off-Site Events & Pilgrimages for Adult Participants & for any Minors Under Their Care: Information & Permission Form is for local field trips, off-site events, pilgrimages, and conferences serves as a legally binding waiver and release of liability, ensuring that both the participant and any minors under their care understand and agree to the terms and conditions of participation in the event. These cannot be used in place of a Event Permission Form for Minor Participant. The adult signing must be present for the duration of the event in order for this to be used for a minor.

• The Local Field Trips, Off-Site Events & Pilgrimages for a Minor Participant: Information & Permission Form is for local field trips, off-site events, pilgrimages, and conferences for minors serves as a legally binding waiver and release of liability, ensuring that both the parent and the participant understand and agree to the terms and conditions of participation in the event.

• El Formulario Anual de Permiso y Liberación General para Menores: Formación Religiosa y Actividades Juveniles otorga permiso para que el participante participe en la formación religiosa y actividades juveniles llevadas a cabo en la Parroquia por un período de doce meses, indicando que el padre/madre/tutor entiende la naturaleza rutinaria de estas actividades. Estos no pueden ser utilizados para ningún evento fuera de la parroquia o la escuela. Ni siquiera para eventos diocesanos u organizados por la Oficina de Juventud.

• El Formulario de Responsabilidad General para Adultos y para Cualquier Menor a Su Cargo establece los términos y condiciones para la participación en el evento, incluyendo exenciones de responsabilidad, reconocimiento de riesgos, consentimiento médico, autorización para el uso de medios, asuntos procedimentales y el compromiso de ser responsables de los menores a su cargo. Estos no pueden ser utilizados en lugar de un Formulario de Liberación de Responsabilidad General para Participantes Menores. El adulto debe estar presente durante todo el evento para que este sea utilizado para un menor.

• El Formulario de Liberación de Responsabilidad General para un Participante Menor establece los términos y condiciones para la participación del menor en el evento, incluyendo exenciones de responsabilidad, reconocimiento de riesgos, consentimiento médico, autorización para el uso de medios y asuntos procedimentales. Estos solo deben ser utilizados para las participaciones únicas de un joven en una actividad parroquial, como un grupo juvenil, clase de formación religiosa, etc. Estos no pueden ser utilizados en lugar de un Formulario de Permiso Anual o Formulario de Permiso para Eventos. No es necesario que el padre/madre/tutor firmante esté presente en el lugar, pero deben ser el tutor legal.

• El Formulario de Información y Permiso para Excursiones Locales, Eventos Fuera del Sitio y Peregrinaciones para Participantes Adultos y para Cualquier Menor a Su Cargo es para excursiones locales, eventos fuera del sitio, peregrinaciones y conferencias, y sirve como una exención de responsabilidad y liberación legalmente vinculante, asegurando que tanto el participante como cualquier menor a su cargo entiendan y acepten los términos y condiciones de participación en el evento. Estos no pueden ser utilizados en lugar de un Formulario de Permiso para Eventos para Participantes Menores.
